What is a CDM plan?

This is a Medicare program where eligible participants (see below for details) are able to get a rebate for up to 5 sessions in a calendar year.

Am I eligible for Medicare rebates?

If you have a chronic medical condition, your GP may refer you for Dietetics services under Medicare’s Chronic Disease Management (“CDM”) program (formerly known as Enhanced Primary Care or EPC).

What is a chronic medical condition?

A chronic medical condition is one that has been (or is likely to be) present for six months or longer, for example diabetes, cancer, heart disease and musculoskeletal conditions including osteopenia and osteoporosis.

Your GP will determine whether a CDM plan is appropriate for you and can refer you for a maximum of five allied health services per calendar year.

How much is the rebate?

The consultation fee is payable as normal on the day of your appointment and the Medicare rebate is $54.60.

For face-to-face appointments the rebate can be processed into your account on the spot. All you need to do is bring your Medicare card and your EFTPOS (Cheque or Savings) card with you.

For Telehealth (Phone or Video) appointments, we can email the invoice to you so you can claim the rebate via your MyGov account or at a Medicare office.

How do I make an appointment once I have a referral?

Once you have received your GP referral, call us on 9570 1552 or click the link below and select Dietetics Initial Consultation CDM – Telehealth appointment.  Please let us know your GP’s details at the time of booking (by phone or in the online booking notes), so we can make sure we receive the referral prior to your appointment.
